Request a prescription for speech therapy from your child's Primary Care Provider (PCP). We will also ask for an updated hearing screening, which your PCP can provide.
2. Request Intake Form

Request an intake form. This form can be filled out in the comfort of your own home through our online portal. We will ask that you complete this form within 5 days.

Once the intake form and all other required documents are received, we will reach out to schedule your evaluation appointment!

DBST primarily provides services to school-aged children with speech-sound disorders, language disorders, and/or language-literacy (reading) disorders.
DBST is currently in-network with Presbyterian (Commercial and Medicaid) and BlueCross BlueShield (Commercial and Medicaid).
If you have a different commercial/private insurance program, we can provide a superbill that you can submit to your insurance for possible reimbursement. Reimbursement is not guaranteed and is dependent on the details of your specific insurance program.
DBST offers a prompt-pay discount for those who are self-pay. Those in this category will automatically qualify as we require payment on the day of service.
The following are necessary before we reach out to schedule your evaluation:
- Physician Prescription - Can fax to (575) 339-2088
- Insurance Card
- Guardian Identification Card
- Intake Form Completion
- Other relevant documents (ex. past speech-language evaluations, IEPs, audiologic evaluations, etc.)
There is currently not a waitlist for evaluation or therapy.
